			<content:encoded><![CDATA[<p>As a result of the current restrictions on healthcare, consumers have begun to assume responsibility for their own health by taking preventive measures through exercise and alternative care options. However, the lack of both know-how and confidence to make health-related decisions has created a demand for leadership in this area. This is where health clubs enter. <span id="more-331"></span></p>
<p>With their professional staff&#8217;s expertise, health clubs can help consumers on their path to achieve optimal health, thereby minimizing the use of the healthcare system. If a visit to a physician&#8217;s office is considered primary care, then a visit to a health club should be considered primary-primary care.</p>
<p>Health clubs have earned their place in healthcare by helping consumers stay healthy and away from illness and injury. They have promoted programs that prevent, control and reverse disease through exercise and promotion of healthy habits (see Figure 2). </p>

<p style="text-align: justify;"><span id="more-296"></span>The chance of dying from prostate cancer depends on the stage and grade of the cancer when it is diagnosed. Men who are treated for prostate cancer that hasn&#8217;t spread outside the prostate can expect to live as long as other men their age.</p>
<p style="text-align: justify;">About 13% of men with prostate cancer that hasn&#8217;t spread beyond the prostate (TMN staging T1 or T2 or Gleason score 2 to 7) die within 10 years of the diagnosis 2.</p>
<p style="text-align: justify;">There is no convincing evidence that treating prostate cancer that hasn&#8217;t spread improves the man&#8217;s survival significantly more than does watchful waiting alone. More study is being done on this question, but the results will not be available for several years.</p>
<p style="text-align: justify;">Younger men diagnosed with prostate cancer who are otherwise healthy have a greater chance of eventually dying of the cancer. Aggressive treatment with the goal of curing the cancer may be appropriate for them. Survival rates are good if the cancer has not spread outside the prostate.</p>
<p style="text-align: justify;">In some cases, prostate cancer may spread (metastasize) outside the prostate into the surrounding tissues, pelvic lymph nodes, and eventually spread into the bones, lungs, or other organs.</p>
<p style="text-align: justify;">Men who have cancer that has spread outside the prostate gland into surrounding tissues but has not spread to distant organs (Jewett system stage C or TMN system T3 to T4) are unlikely to be cured. Younger men with this advanced cancer often die from the cancer. However, the time before this happens is highly variable and may be up to 5 years or longer. Therefore, older men may die of other health problems before the cancer can kill them.</p>
<p style="text-align: justify;">Cancer that is detected after it has spread outside the prostate gland into the lymph nodes or the bones (Jewett system stage D or TMN system stage N1 to N3 or any M) cannot be cured, and survival rates are poor. Most men live 1 to 3 years after this diagnosis, and die from the cancer. However, a few men may survive many years, even with advanced cancer.</p>

Cholesterol is essential to the body. Cholesterol is a precursor to many hormones and acts as an insulator to nerve fibers. Cholesterol is a normal substance that is found in many animal based foods and is also produced by the liver. There are numerous factors that influence cholesterol levels. Diet, genetics, smoking, age and gender.</p>
<p style="text-align: justify;">There are several types of cholesterol and some are beneficial. It is not only the amount of cholesterol that is important but also the ratio between the beneficial and harmful types. Cholesterol is transported throughout the body by lipoproteins. Beneficial cholesterol is often referred to as high-density lipoprotein or HDL and harmful cholesterol as low-density lipoprotein or LDL. A cholesterol test should tell to the amounts of total cholesterol as well as total LDL and HDL, and total triglycerides.</p>
<p style="text-align: justify;">Over the years nutritional researchers are refining our understanding of the role that cholesterol plays in the development of atherosclerosis or hardening of the arteries. Cholesterol forms a plaque in the artery walls that builds up over time. The narrowing of the arteries and the stiffening of the arteries restricts blood flow and increases the odds of having a heart attack or stroke.</p>
<p style="text-align: justify;"><strong>A Natural Approach </strong><br />
There are numerous lifestyle and nutrient choices that can be utilized in a natural approach to cholesterol maintenance. The first factors to consider are diet and exercise. Consumption of large amount of dietary fats and refined sugars will increase cholesterol levels. Everyone concerned with high cholesterol levels should become knowledgeable regarding the dietary and lifestyle factors that are within their control and make choices that reduce their risk factors. In this article I will be focusing on the supplements that one can take to improve cholesterol levels.</p>
<p style="text-align: justify;">When using a natural approach to maintain healthy cholesterol levesl it is important to take a holistic approach. A holistic approach that includes a variety of nutrients that taken together will provide much better results than any single nutrient. Therefore a holistic approach will consider several nutrients that serve not only to lower cholesterol levels but to also reduce the damage that occurs in the body.</p>
<p style="text-align: justify;"><strong>Anti-Oxidants</strong><br />
When cholesterol loses electrons to oxygen, it becomes oxidized and changes properties. This reaction can occur when cholesterol rich foods are being cooked or processed as well as in the body in the LDL particles in the blood. This process is call lipid peroxidation and researchers have confirmed that oxidized cholesterol is much more adhesive and is more likely to form plaque on arterial walls causing further damage.</p>
<p style="text-align: justify;">Increasing the levels of anti-oxidants is an important step in reducing the damage caused by oxidized cholesterol. One of the most beneficial anti-oxidants is vitamin E. Natural vitamin E has been shown to prevent and reduce the oxidation of cholesterol. It would be wise to consider an anti-oxidant complex that includes such nutrients as lipoic acid, grape seed extract, glutathione, N-Acetyl cysteine and beta carotene.</p>

The use of niacin in high potencies has been shown to be an excellent approach to cholesterol management, however the side effects of niacin can be problematic. There is concern that high levels of niacin can contribute to liver problems.</p>

<p> You will be acutely aware of the chronic shortage of supported accommodation for people with intellectual disabilities in NSW. </p>
<p> Not only are carers having to provide the supported accommodation which Government is unwilling to fund, but the service system is so stretched that people are pushed into crisis before the system will respond. <span id="more-269"></span></p>
<p> Many carers are faced with the unacceptable choice of continuing to provide care for their disabled relatives with little or no assistance and at great stress, or leaving their disabled family member at a respite facility and not picking them up. </p>
<p> The scale and intensity of this problem is of epidemic proportions: at least 20 per cent of respite facilities are unavailable due to inadequate funding, and at least 1,500 people with disabilities require – but cannot access – ongoing supported accommodation in this State. </p>
<p> Unfortunately, the system is presently geared to managing political ‘noise’ rather than providing fair and participatory outcomes for people with disabilities and their carers. </p>
<p> There is no foreseeable improvement in this situation, so that unless something changes, strains in the care system and family stresses will continue to grow over time. </p>
<p> This can not be allowed to continue. Disabled peoples’ and carers’ pain has been privatised long enough. It is now time to give voice to the reality of the situation for people with disabilities and their carers and turn this situation around. </p>
<p> An End to the Silence is the name of a project which documents the scale of this crisis situation. </p>
<p> Its main focus is a report which will pull together in one volume the many stories of people with intellectual disabilities and their families in the context of the supported accommodation crisis. </p>
<p> In telling these stories, carers’ contributions will be articulated and acknowledged, and pressure brought to bear on the NSW Government to provide sufficient funding for supported accommodation. </p>
<p> Accordingly, I am asking you to tell your story – in one page – so that I can collate it with many hundreds (and possibly thousands) of others into the End to the Silence Report. The Report will be publicly launched and presented to the NSW Government in mid-October 2005. It will also be the centrepiece of a media campaign designed to highlight this issue. The Report will be supplemented by data and statistics on the supported accommodation shortfall. </p>
<p> Each page of the collated report will tell one family’s story. It will both record the reality of the situation, at the same time as acknowledging carers’ and family members’ contributions to the community through their care for their disabled family member. </p>
<p> Please send me the story of your family to include in this Report. You are free to express this as you wish, although your contribution must not exceed one A4 page in length. You may like to include the following:<br />

 how long you have cared for your disabled family member<br />

 what the high points – or the challenges and low points – are in your day<br />
