Expert advisors to the US Food and Drug Administration (FDA) recommended the approval of 30-day extended wear contact lenses made by Ciba.

The Ophthalmic Devices Panel’s vote signals that the extended wear lenses may soon be available in the US. The Focus Night and Day lenses would be the first 30-day lenses available in this country.

Although the FDA is not legally bound to abide by its panels’ recommendations, the agency generally does follow the panels’ advice.

If approved, the lenses would give millions of nearsighted and farsighted Americans the option of leaving in their lenses for a full month, without cleaning and disinfecting.

At present, the extended wear lenses approved in the US are limited to 7 days of continuous wear.

Unlike these lenses, the Focus Night and Day contact lenses are made from Lotrafilcon A, which allows up to six times more oxygen to reach the cornea than standard soft lenses. Lack of oxygen to the eyes is the primary reason most lenses cannot be worn overnight.

The panel’s vote was based upon clinical data gathered from about 660 clinical trial participants, in which it was demonstrated that lenses resulted in 20/30 visual acuity in about 99% of the treated eyes when worn over an average of 27 to 28 nights.

The study compared the 30-day lenses to Johnson & Johnson’s Acuvue 7-day extended wear lenses.

This data also showed that the lenses resulted in statistically significant less drying. However, the panel expressed concern about allowing the company to make that claim because it may be construed that the lenses are appropriate for people who have problems wearing the 7-day lenses.

“Patients should understand what we mean by safety and efficacy,” noted panel member Dr. Karen Bandeen-Roche, a professor of biostatistics at John Hopkins University in Baltimore, Maryland.

The panel also expressed concern about the possible incidence of corneal ulcers as well as the seemingly higher rate of inflammation on the membrane that covers the outer layer of the eye and inner layer of the eyelids in those who wore the new lenses.

Traditionally, the FDA has allowed an incidence of about 20 corneal ulcers per every 10,000 people. But although the Night and Day lenses were shown to be identical to the 7-day extended wear lenses in terms of this side effect, the panel observed that there was still insufficient data to establish whether the 30-day lenses would result in a higher rate of ulceration.

Still, the panel decided to vote in favor of approval on the condition that the company removed the “dryness” indication from the label, outlined the risk of inflammation and conducted a post-marketing study to determine the incidence of corneal ulcers.

Despite these requirements, “I think the sponsor [Ciba Vision] demonstrated that it would be an effective alternative for people that want extended wear lenses,” said Dr. Jayne Weiss, a professor of ophthalmology at Wayne State University in Detroit, Michigan.

“As a practitioner, I truly welcome this new advance,” added Dr. Alice Matoba, an associate professor of ophthalmology at Baylor College of Medicine in Houston, Texas.

The most commonly reported side effects leading to the removal of the lenses were irritation, poor fitting lenses and dryness.

Additional companies that are expected to seek approval of 30-day extended wear contact lenses include Johnson & Johnson and Bausch and Lomb.

Ciba Vision is a subsidiary of Novartis AG, a healthcare conglomerate based in Switzerland. The Focus Night and Day lenses currently are available in approximately 40 countries worldwide.

We know from experience that sexual dysfunction may ruin a relations, but there are strong, effective and safe remedies for premature ejaculation in our time. PE or Rapid ejaculation is the most common form of sex problem in guy younger than forty years.

Questions,
- in an editorial, did Mr. Ornery name any names?
- did he condemn lotteries?
- did he oppose pay-for-guaranteed play service?
- did he indicate in any way that he would, to borrow wrestler Chris Jerrico’s old tag line, “never…EVER…” play games of chance again?

The answer to each question is: no he did not.

Mr. Garry condemned misleading advertising, promotional material that starts off one way and leads to something else. Any sane person does or should. Yet the shysters of the world persist in many forms and guises.

The mentality seems to be: create a product or service and THEN create a demand for it. Mislead. Deceive.

Just a curiosity but when did it become accepted advertising practice to mislead or to lie blatantly? Can anyone truly believe much of anything that inundates the eyes and ears anymore? Granted, there are some companies that back what they say with facts, figures, and research. Too many though say one thing, mean something else entirely, and bury the truth in fine print requiring a magnifying glass to read.

Who could imagine that a simple rant against a deceptive sales tactic could result in Mr. Ornery’s apparent blacklisting from that very game he has played off and on for over a year? That seems to be the case. Attempts to log in and play in the usual way were met with: “Sorry. There was an error in processing your request. Please try again.”

This is a first. Still, it is nice to know people read these words, and better still when they start paying attention. Best of all, in this case will be when the company ceases to mislead, and judging by an e-mail this morning, it is starting to do that. Time will tell. Meanwhile, the folks at nameless lottery corporations should really observe honorable advertising standards and also let in players who have ‘never…EVER’ declared that they would ‘never…EVER’ play again.

The pattern of drug errors is increasing, and there are actions you can take to prevent dangerous problems from arising due to prescription drugs in your house. First, flush down the toilet any outdated medications, especially those that are discontinued. Discard all eye and nose medications after your course of treatment, and do not reuse them. Read the label on the bottle and check the expiration dates; even if the medication appears okay, most medications lose their potency after this date. A lot of people get prescriptions from their doctor and don’t ask for any instructions except how often they should take the pills. Ask your doctor these questions: Am I taking anything that will not work well in conjunction with this medication? How long do I have to take this medication? How often must I take it? What side effects should I watch for? When does this medication begin working? How will I know it’s working? Inquire about what can be done to see if the medication is working, like taking blood pressure or blood sugar levels. Be very careful that you don’t crush any tablets that have a sustained release action, as this could deliver the whole dose at once and cause a drug overdose. It is important to know what to do if you miss a dose, and never take more than one dose at a time. Some of the above questions can be answered by using your pharmacy. Many times people will stop taking their medicines when they start to feel better; men will stop taking blood pressure medications when they begin to feel better or will stop hypertension medications because they can cause erectile dysfunction. Although substitution medication might be available, it is necessary to consult your physician before stopping any medication – good communication is the key! When you stop taking your antibiotics before you have completely completed the course, you are helping “bugs” become “super-bugs”. They build up an immunity to antibiotics because of this incomplete exposure and become very difficult to treat. New antibiotics are always being developed to treat these infections, and you may require a longer treatment regimen. Physicians try to use antibiotics only in case the infection is bacterial and not viral, however, many the people insist the physician order some type of medication. A viral infection, like the common cold, are generally resolved within a short time frame. Our society depends on taking pills to feel better instead of trying some alternative treatments which have been proven to work. You can find many alternative treatments online. I do recommend that you speak with your doctor and draw up a treatment plan. It is extremely crucial to make your doctor and pharmacist aware of all drugs you are taking which includes anything that you get over the counter. Medications made out of many herbal ingredients have the capacity to react negatively with prescription medications, and often red flags appear on computers to make the physician and pharmacist identify the possible error. Sometimes a person may be taking more than one product that thins the blood. For example, say you are already using Tylenol or a prescription narcotic, then you begin taking a prescription blood thinner. The result could be permanent liver damage. One way to eliminate medication mistakes at home is to purchase a pill organizer. If you have problems with your vision, have a friend or relative help you, and get pill containers that are easy to read. Never put different pills in the same bottle, because this might lead to mistakes. Set yourself up a schedule for taking your medications. Keep in mind that you will need to take some medications with food and others without food. There are also some foods you will need to avoid while taking some medications. For example, if you are taking heart medications, steer clear of grapefruit juice. Also remember that some medications have negative interactions. Avoid taking these together. Be sure to read the valuable information contained on prescription medication labels. Phone your pharmacist with your questions; they are eager to help you. Generic medications are the equivalent of brand name prescription medications. They are the proper substitutes for their brand name counterparts and should not be considered inferior. Insurance companies do prefer that their clients use cheaper generic medication, and hence the resulting payments are found to be less in the case of a claim. Some of the names are tough to say, but if you know the name of what you want to substitute, it is okay to use that name. It is not wise to memorize medications in terms of its color or other characteristics since in the case of substituted medicines, the medication may have a different appearance and lead to patient confusion. If you will take a little time to learn and ask questions, you can prevent making mistakes with medications in the home. The transaction is not over just because you get a prescription. Communication of any concerns, and a detailed understanding of what each prescription does, may prevent a number of medication interaction errors from occurring.
